Locations: Livermore & El Sobrante, CA | $31.00 – $46.50/hour
Schedule: Full-Time | Monday–Friday, 8am–5pm
Department: Geriatric Rehabilitation | PACE Program

The Role
Under the supervision of a Physical Therapist, you will:
- Implement individualized treatment plans focused on safety, independence, and functional mobility
- Provide therapeutic interventions including exercise, gait training, neuromuscular re-education, manual techniques, and physical modalities
- Educate participants and caregivers on assistive devices, mobility strategies, and adaptive equipment
- Lead and assist with group therapy sessions
- Collaborate with an interdisciplinary care team to ensure coordinated, high-quality care
- Document services accurately and maintain therapy equipment and supplies
- Travel locally to participant homes and multiple program sites as needed
Qualifications
- Associate’s degree from an accredited Physical Therapist Assistant program
- Current California PTA license (required)
- CPR and First Aid certification
- Valid California driver’s license, current vehicle insurance, and reliable transportation
- Experience with frail elderly or geriatric populations preferred
- Strong communication skills and collaborative approach
