Minimum Qualifications
- Associate degree in Physical Therapy from an accredited educational program.
- Active AZ State PTA license is required to start the assignment.
- BLS certification from AHA or ARC may be required prior to start.
- Experience working in a skilled nursing facility or long-term care setting preferred.
- Experience with common SNF equipment and procedures preferred, including therapeutic exercise programs, gait training, transfers and mobility devices, and restorative care documentation.
- Ability to meet productivity expectations as required by the facility. Net Health/Rehab Optima will accept COVID exemptions; a current resume is required to submit for credentialing and review.
