Start Date: ASAP | Duration: 13 Weeks
Schedule: 40 hrs/week | 5x8s | 8:00 AM – 5:00 PM

Provide high-quality rehabilitative care to residents in a skilled nursing facility, supporting mobility, function, and overall quality of life.

Responsibilities:

• Evaluate, plan, and implement individualized treatment programs
• Collaborate with the care team for comprehensive patient care
• Document patient progress and adjust treatment plans as needed
• Educate patients and families on therapy goals and exercises

Requirements:

• Current PT license
• Minimum 1 year of PT experience (SNF experience preferred)
• Current BLS certification
• Strong clinical skills and ability to work independently
