Become a Home Health Physical Therapist at CenterWell Home Health
As a Home Health Physical Therapist reporting to the Clinical Manager, you'll help patients regain strength, mobility, and independence in the comfort of their own homes. Through personalized, goal-oriented care, you'll empower patients to overcome limitations, improve function, and enhance their quality of life.
Responsibilities
Plan and administer prescribed physical therapy treatments in patients' homes to help them achieve their highest level of physical function
Assess and document patients' physical strengths, limitations, and progress to support physician evaluation and treatment planning
Develop and implement individualized rehabilitation programs consistent with the physician's plan of care and patient goals
Provide clear care instructions to Physical Therapist Assistants and Home Health Aides and oversee their performance and documentation
Accurately document patient assessments, interventions, and outcomes
Communicate patient progress to physicians, the Clinical Manager, interdisciplinary care team, patients, and families
Complete and submit all required evaluations, treatment plans, progress notes, and discharge summaries in a timely manner
Required Qualifications
Current, unrestricted Physical Therapy license
Current CPR certification
Experience collaborating with an interdisciplinary healthcare team
Preferred Qualifications
One year of experience as a Physical Therapist
Previous home health experience preferred

TB Screening Requirement This role is considered patient-facing and is part of Humana's Tuberculosis (TB) screening program. Candidates selected for this position will be required to complete TB screening.
