Work Environment
This full-time position is onsite, primarily working in home care settings throughout Lakewood and the surrounding areas. Key Responsibilities
- Conduct comprehensive evaluations to assess patients' physical capabilities and design tailored therapeutic plans to meet individual goals.
- Deliver compassionate and skilled therapy services, including therapeutic exercises, manual therapy, and various modalities appropriate for the geriatric population.
- Regularly monitor and document patient progress, making pertinent adjustments to treatment plans to maximize outcomes.
- Collaborate with a multidisciplinary team of healthcare professionals—including physicians, occupational therapists, and speech therapists—to ensure holistic patient care.
- Educate patients and their families on therapeutic interventions, home exercise regimens, and strategies for injury prevention.
- Implement and oversee fall prevention programs, ensuring the safety and well-being of our patients in their homes.
- Stay abreast of the latest developments in physical therapy practices and engage in ongoing education to enhance clinical skills and knowledge.
- Maintain the highest ethical standards of care, uphold patient confidentiality, and consistently demonstrate professionalism in all interactions.
Patient Population / Client Focus
Our primary focus is the geriatric community, which requires a tailored and empathetic approach to therapy. Patients often face unique challenges including mobility limitations, chronic conditions, and cognitive changes associated with aging. As a Physical Therapist at Motion Craft Wellness, you will be instrumental in facilitating the rehabilitation and independence of these individuals by providing targeted, evidence-based interventions.
Qualifications
- Must hold a Bachelor's, Master's, or Doctorate degree in Physical Therapy from an accredited institution.
- An active State licensure as a Physical Therapist is required.
- Strong interpersonal and communication skills are essential for effective engagement with patients, families, and health care colleagues.
- Proficiency in electronic medical records (EMRs) for accurate patient documentation and record-keeping.
- Ability to apply evidence-based practices and current therapeutic techniques to achieve optimal patient outcomes.

Preferred Experience

Compensation and Benefits
The salary range for this full-time position is set between $100,000 and $120,000 annually, reflecting the experience and expertise of our therapists.
