Work Environment
This role is based onsite in Oswego, Illinois, primarily within the home care setting, allowing you to engage directly with patients in the comfort of their own environments. Key Responsibilities
- Patient Evaluations: Conduct thorough assessments of patients' physical abilities, understanding their unique challenges and goals to develop tailored treatment plans.
- Therapeutic Interventions: Provide skilled therapy services such as manual therapy, therapeutic exercises, and appropriate modalities to enhance patient recovery.
- Progress Monitoring: Regularly document and evaluate patient progress, making necessary adjustments to treatment plans to ensure optimal outcomes.
- Multi-Disciplinary Collaboration: Work with a team of healthcare professionals, including physicians and occupational therapists, to develop comprehensive care strategies.
- Patient Education: Empower patients and families with knowledge about therapeutic techniques, home exercise programs, and strategies for preventing injuries.
- Safety Measures: Implement and promote fall prevention programs and safety measures within the assisted living environments to safeguard patients' well-being.
- Professional Development: Stay informed about the latest advancements in physical therapy through continuous education and training opportunities.
- Ethical Standards: Adhere to ethical and professional standards while maintaining patient confidentiality and trust in the therapeutic relationship.
Patient Population / Client Focus
Your primary focus will be our geriatric clientele who often face complex physical challenges due to age-related conditions. Working with this demographic requires patience, understanding, and a tailored therapeutic approach. The goal is to enhance their mobility, reduce pain, and improve overall quality of life by fostering independence through rehabilitation.
Qualifications
- A degree in Physical Therapy from an accredited college or university, with a Doctorate, Master's, or Bachelor's preferred.
- Active state licensure to practice as a Physical Therapist.
- Strong communication skills, enabling effective interactions with patients, their families, and the multidisciplinary healthcare team.
- Proficiency in electronic medical records (EMRs) for accurate documentation and patient management.
- Understanding of evidence-based practices in physical therapy, applying the latest therapeutic techniques to improve outcomes.
- Dedication to patient-centered care, focusing on enhancing quality of life through supportive and individualized therapy.
- A track record of both independent work and collaboration within a team environment.
Preferred Experience
- Prior experience working in geriatric therapy settings is preferred; however, motivated new graduates are also encouraged to apply.
- A proven ability to execute patient care plans effectively while demonstrating initiative in therapeutic interventions and client relations.
- Knowledge of current best practices and trends within geriatric physical therapy enhancements.
Compensation and Benefits
