Work Environment
This role involves working onsite, primarily in a home care environment where you will interact with residents in their living spaces. Key Responsibilities
- Conduct thorough physical therapy evaluations to determine residents' mobility, strength, and functional abilities, using standardized tools and assessments.
- Design and implement personalized treatment plans that address individual needs, focusing on mobility improvement, fall prevention, and daily functional enhancement.
- Utilize a variety of therapeutic techniques such as manual therapy, therapeutic exercises, and the use of assistive devices to support improvement in residents' physical performance.
- Educate residents and their families on rehabilitation strategies and exercises, ensuring they understand the importance of continued engagement in their recovery process.
- Monitor and document residents' progress meticulously, adjusting therapy plans based on ongoing assessments and feedback from the residents and their care team.
- Lead integrative team meetings to discuss patient goals and review therapeutic outcomes, ensuring all team members are aligned in their approach to care.
- Maintain comprehensive and accurate clinical documentation that adheres to healthcare regulations and standards, facilitating optimal communication with all healthcare stakeholders.
Patient Population / Client Focus
In this role, you will be primarily working with senior residents in assisted living arrangements, many of whom may face challenges related to mobility, chronic pain, and age-related functional decline. The ability to engage empathetically with this demographic is paramount, as you will be addressing their unique physical therapy needs while fostering a supportive and understanding environment.
Qualifications
- A Bachelor's degree in Physical Therapy from an accredited institution is required; advanced degrees such as a Doctorate (DPT) or Master's degree are highly preferred.
- An active Physical Therapy license in Oregon is essential for practice.
- A minimum of 2 years of experience in a geriatric or assisted living environment is essential for success in this role.
- Proficiency in current treatment methodologies related to mobility enhancement and pain management.
- Strong communication and interpersonal skills to effectively engage with senior residents and collaborate with multi-disciplinary team members.
- Ability to function independently while also contributing as a valuable team member in a comprehensive care environment.
- Familiarity with electronic health record (EHR) systems and meticulous attention to detail in documentation.
Preferred Experience

- Exposure to home care practices and understanding the complexities of working in varied living environments.
