Work Environment
As a Physical Therapist in our home care facility, you will work primarily with elderly patients within a serene assisted living community. This onsite position enables you to take advantage of modern facilities, including dedicated therapy rooms, gym spaces, and comfortable areas for patient-relaxed engagement. You will collaborate with a multidisciplinary team of healthcare professionals, ensuring a comprehensive approach to patient wellness.
Key Responsibilities
As a Geriatric Physical Therapist, your primary responsibilities will include:
- Conducting thorough assessments to identify each patient's individual rehabilitation goals and needs.
- Administering effective treatment plans that promote enhanced mobility and movement, utilizing techniques tailored to the geriatric population.
- Collaborating with healthcare providers to develop and implement care plans addressing patients' unique needs.
- Utilizing evidence-based practice to devise customized fitness and wellness programs aimed at maximizing patients' physicality and well-being.
- Monitoring progress through consistent evaluations and effective communication with patients and their families.
- Documenting patient care services accurately to adhere to compliance regulations and ensure continuity of care.

- Teaching patients safe practices to reduce dependence on medication and providing alternatives to invasive procedures.
Patient Population / Client Focus
Your role will focus primarily on the geriatric population, addressing the unique physical and rehabilitation challenges faced by this age group. You will engage with patients suffering from age-related ailments such as arthritis, balance disorders, post-surgery recovery, and chronic pain. Your compassionate approach will help patients regain independence, mobility, and overall health, enhancing their quality of life.
Qualifications
The ideal candidate will possess the following educational and professional qualifications:
- A Bachelor's Degree in Physical Therapy from an accredited institution.
- A valid state license to practice physical therapy.
- A strong understanding of treatment practices relevant to geriatrics, orthopedics, and physical rehabilitation medicine.
Preferred Experience
While we welcome recent graduates, candidates with prior experience in a home care setting or directly working with the elderly population are preferred. Experience with current rehabilitation technologies, treatment methodologies, and interdisciplinary collaboration will be highly regarded.
Compensation and Benefits
