As a member of our onsite team, you will work closely with fellow healthcare professionals, including occupational therapists, nurses, and case managers, in a setting that encourages interdisciplinary communication and comprehensive patient assessments. Key Responsibilities
As a Physical Therapist at Revive Motion Clinic, you will undertake the following responsibilities:
- Design and implement individualized treatment plans based on thorough assessments to prevent disability and relieve pain effectively.
- Oversee and mentor physical therapy assistants to ensure the accurate execution of the care plans.
- Conduct detailed evaluations of clients' functional levels using diagnostic tests that assess muscle, nerve, and joint capabilities.
- Evaluate patients' home environments and recommend modifications to promote independence and enhance rehabilitation outcomes.
- Administer prescribed physical therapy services in clients' homes, performing manual therapeutic exercises and functional activities.
- Perform traction, massage, and administer treatments involving physical agents as part of the therapeutic process.
- Fit, adjust, and evaluate prosthetic and orthotic devices, advising physicians on necessary modifications.
- Prepare comprehensive clinical documentation, including evaluations, care plans, progress notes, and discharge summaries.
- Engage in case conferences as a key member of the interdisciplinary team, contributing to the overall treatment strategy and patient care discussions.
- Provide education to clients and their families regarding their physical therapy programs, including the use and care of devices.
- Establish ongoing assessments and adjust care plans as necessary to meet evolving patient needs.
Patient Population / Client Focus
The ideal candidate will focus on diverse patient populations, including those recovering from surgery, injury, or illness. Our clients range from pediatric to geriatric individuals, often with varying physical challenges. You will play a key role in supporting their rehabilitation journey by offering personalized care strategies aligned with their unique needs and circumstances.
Qualifications
To be successful in this role, candidates should possess the following qualifications:
- Graduation from an accredited physical therapy program.
- A minimum of one year of experience in a physical therapy role, preferably in an acute care or home health setting.
- Current PT licensure in good standing in the state of Vermont.
- Active CPR certification.
- A valid driver's license and automobile insurance.
- Strong clinical competencies and a commitment to delivering high-quality patient care.
- Excellent communication skills and the ability to collaborate effectively with multidisciplinary team members.
Preferred Experience
While not mandatory, candidates with experience in home health therapy services or those who have worked with varied age groups will be highly regarded. Familiarity with electronic medical records (EMR) systems and experience in developing home-based rehabilitation plans will give applicants a competitive edge.
Compensation and Benefits
This position offers a competitive salary range of USD 100,000 to 120,000 per year, commensurate with experience.
