Work Environment
This position requires working within patients' homes, adapting therapy approaches based on each individual's environment and needs. You will collaborate with other healthcare professionals, including physicians and registered nurses, to provide comprehensive care plans. The role involves assessing patient functionality and adjusting therapeutic exercises on-the-fly to accommodate varying home settings and patient capabilities.
Key Responsibilities
- Perform thorough evaluations of patients, identifying physical impairments, disabilities, and limitations to formulate personalized treatment plans aligned with physician directives.
- Develop and implement treatment programs tailored to each patient, incorporating therapeutic exercises, pain management strategies, and mobility enhancement techniques.
- Maintain meticulous documentation of patient evaluations, progress notes, treatment plans, and discharge summaries to ensure compliance with healthcare regulations and share findings with the healthcare team.
- Analyze therapy outcomes to determine the effectiveness of interventions, modifying treatment plans as necessary while collaborating with physical therapy assistants and support staff for optimal patient care.
- Provide supervision and mentorship to Physical Therapy Assistants, ensuring they are equipped to support patient care strategies effectively.
- Regularly interact with other healthcare professionals, exchanging patient insights to enhance treatment efficacy and ensure a multidisciplinary approach to care.
- Engage in continuous professional development to remain current on best practices and emerging trends in physical therapy.
Patient Population / Client Focus
The ideal candidate will engage with a diverse client group, including elderly patients recovering from surgery, individuals with chronic pain conditions, and those needing rehabilitation post-injury or illness. This role demands empathy, effective communication, and the ability to adapt therapy practices to accommodate the varied needs and limitations of patients in their home environments.
Qualifications
- Bachelor's Degree in Physical Therapy from an accredited institution.
- Current state licensure as a Physical Therapist in Vermont.
- Excellent communication skills, both verbal and written, to effectively document patient interactions and collaborate with healthcare teams.
- Proficiency in utilizing technology and electronic health records for documentation and treatment tracking.
Preferred Experience
- At least one year of experience working as a Physical Therapist, particularly in a home health or rehabilitation setting is preferred.
- Experience using therapeutic modalities and specialized equipment to aid rehabilitation processes.
Compensation and Benefits
