Pinnacle Rehab Associates is actively seeking a dedicated and licensed Physical Therapist to join our outpatient team in Burlington, Vermont. This full-time position is focused on delivering expert physical therapy services to enhance the quality of life for our patients. As a Physical Therapist, you will play a critical role in assessing, developing, and implementing personalized treatment plans to help patients achieve their rehabilitation goals.

Work Environment
This position is located in our outpatient office in Burlington, where you will work in a collaborative team setting alongside other rehabilitation professionals. Key Responsibilities
- Design and implement individualized treatment plans for patients based on thorough assessments and evaluations of muscular, functional, and flexibility capabilities.
- Direct and oversee Physical Therapy Assistants, ensuring they effectively implement prescribed therapy plans.
- Evaluate patients' home environments and recommend modifications that foster independence and safety.
- Administer various physical therapy modalities, including manual therapy, therapeutic exercises, and functional training.
- Conduct diagnostics and apply tests to evaluate patient progress and adjust treatment plans accordingly.
- Document detailed clinical records, generating initial evaluations, care plans, progress notes, and discharge summaries for each client.
- Attend interdisciplinary team meetings to discuss comprehensive care plans and collaborate with other healthcare providers.
- Train clients and families on the use and management of physical therapy devices/orthotics and educate them on their therapy programs.
- Participate actively in case conferences, providing insights and observations that contribute to the patient's care and recovery.
- Maintain compliance with branch policies by submitting timely clinical and payroll documentation.
Patient Population / Client Focus
Our patients range from orthopedic recovery cases to individuals needing rehabilitation following surgeries, injuries, or chronic illnesses. You will work closely with diverse populations, each requiring tailored intervention to achieve optimal recovery and independence. Building rapport with patients and their families is essential in our approach, fostering trust and enhancing the therapy experience.
Qualifications
- A degree from an accredited physical therapy program.
- Minimum of one year of experience in a physical therapy role, preferably in an acute or home health care setting.
- Active Physical Therapist license in Vermont, along with current CPR certification.
- A valid driver's license and automobile insurance are required for home visits.
- Strong decision-making skills and the ability to manage challenging clinical scenarios effectively.
- A passion for delivering clinical excellence and a commitment to patient-centered care.
Preferred Experience
- Experience with developing and executing comprehensive treatment plans that address a variety of conditions.
- Familiarity with electronic medical records systems and experience in a team-based healthcare environment.

Compensation and Benefits
The salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
