Work Environment
This position is based primarily onsite in patients' homes in Burlington and surrounding areas. The home care environment allows therapists to assess and treat patients where they are most comfortable, tailoring programs to fit their everyday life. You will work alongside a team of healthcare professionals, including occupational therapists, speech-language pathologists, and nursing staff. Regular evaluations and assessments will help formulate ongoing care plans to achieve optimal patient outcomes.
Key Responsibilities
- Conduct thorough evaluations of patients to assess their physical function, impairments, and disabilities, determining the most effective treatment interventions.
- Develop personalized treatment plans that incorporate therapeutic exercises, functional training, and other modalities according to physician orders.
- Maintain accurate and up-to-date patient records, including treatment notes, progress assessments, and discharge summaries, in compliance with legal and organizational standards.
- Educate patients and their families on home safety, adaptive techniques, and the importance of adherence to therapeutic exercises for optimal recovery.
- Collaborate with interdisciplinary teams, including physicians, nursing staff, and case managers, to optimize patient care strategies and ensure holistic treatment approaches.
- Supervise and mentor physical therapy assistants and support staff, ensuring they are effectively contributing to patient care and adherence to treatment protocols.
- Stay current with the latest evidence-based practices in physical therapy and engage in continuous professional development opportunities.
Patient Population / Client Focus
You will primarily work with diverse patient populations, including those recovering from orthopedic injuries, neurological conditions, post-operative care, and chronic illnesses. Understanding the unique challenges faced by each individual, you will tailor your approach based on their specific needs, functional limitations, and personal goals, thereby fostering a supportive environment that empowers patients to take charge of their health.
Qualifications
- Bachelor's degree in Physical Therapy from an accredited institution.
- Current license to practice as a Physical Therapist in the State of Vermont.
- Strong written and verbal communication skills, with the ability to foster rapport with patients and their families.
- Proficient computer skills, including experience with electronic health record (EHR) systems.
Preferred Experience
- A minimum of one year of clinical experience in a physical therapy setting, preferably in home health or outpatient care.
- Experience working with an interdisciplinary healthcare team to develop comprehensive care plans and treatment outcomes.
Compensation and Benefits
