Work Environment
This position is onsite at our affiliated assisted living facilities in and around Elmwood Park. You will work within a supportive, cohesive team that values collaboration and communication. Our rehabilitation facility is designed to provide a therapeutic, welcoming atmosphere for both our patients and staff, ensuring that everyone feels at home. Our typical work hours are from Monday to Friday, 9 AM to 5 PM, allowing you to maintain a healthy work-life balance.
Key Responsibilities
- Deliver high-quality, on-site physical therapy services to residents within assisted living communities, tailoring treatments to meet individual needs.
- Conduct thorough evaluations of new patients to develop personalized orthopedic-informed treatment plans that take into consideration their unique health profiles and rehabilitation goals.
- Collaborate closely with facility nursing staff, caregivers, and our internal team, including Orthopedic MDs, Occupational Therapists, and Chiropractic Doctors, to ensure comprehensive care.
- Educate residents and their families on safe mobility practices and home exercise programs, empowering them to take an active role in their recovery.
- Utilize mobile electronic medical record (EMR) tools to document patient progress efficiently and securely, ensuring compliance with HIPAA regulations.
- Act as a key liaison between the assisted living facilities and our multidisciplinary clinic to facilitate patient referrals and follow-up care.
Patient Population / Client Focus
You will primarily work with older adults residing in assisted living facilities, many of whom may be dealing with orthopedic conditions, mobility issues, and chronic pain. This role allows you to make a significant impact on their lives by helping them regain independence and improve their overall quality of life through tailored rehabilitation programs.
Qualifications
- A valid Physical Therapist license in the State of New Jersey (or eligibility for licensing).
- A Bachelor's Degree in Physical Therapy or a related field.
- At least 2 years of experience preferred, especially in outpatient, geriatrics, assisted living facilities, skilled nursing facilities (SNF), or home healthcare settings.
- Excellent interpersonal and communication skills to interact effectively with patients, families, and team members.
- Strong clinical understanding of orthopedic conditions prevalent among aging adults.
- Demonstrated ability to work autonomously in a residential care environment.
Preferred Experience
- Experience working with a multidisciplinary team of healthcare professionals.
- Knowledge of evidence-based practices in geriatric physical therapy.
- Familiarity with mobile EMR systems and their use in practice.
Compensation and Benefits
We offer a competitive salary ranging from $90,000 to $100,000 per year, commensurate with experience. Additionally, you will be eligible for comprehensive health, dental, and vision benefits, paid time off, and holiday pay.
