Work Environment
This role is based onsite at our state-of-the-art assisted living facility in Wilsonville, Oregon, a vibrant community located just a short drive from Portland. Key Responsibilities
Conduct thorough evaluations of residents to identify their physical therapy needs, establishing baseline models of treatment and progress.
Develop and implement individualized treatment plans focusing on improving functional mobility, strength, balance, and pain management.
Guide residents through targeted therapy sessions, which may include exercises for gait training, functional mobility, and fall risk reduction.
Assist residents transitioning from acute care facilities, developing a tailored strategy to support their recovery and reintegration into assisted living.
Collaborate with nursing staff, caregivers, and other therapy disciplines to ensure comprehensive care and support for each resident.
Educate caregivers and family members on effective techniques to assist residents in physical tasks and safety protocols.
Maintain accurate and meticulous documentation regarding patient progress in alignment with Medicare guidelines and facility policies.
Patient Population / Client Focus
Your primary focus will be the senior population residing in our assisted living facility, including those with various musculoskeletal, neurological, and mobility-related conditions. Many residents will require tailored interventions aimed at enhancing their quality of life, managing chronic pain, or recovering from surgical procedures. By applying clinical best practices, you will contribute significantly to their physical and emotional well-being.
Qualifications
Bachelor's degree in Physical Therapy from an accredited program (DPT, MPT, or BSPT).
Active Oregon Physical Therapist license or eligibility for licensure.
Exceptional interpersonal skills and a demonstrated ability to communicate effectively with older adults and their families.
A strong commitment to high-quality patient care, including patience, empathy, and a passion for senior health.
Preferred Experience
At least 2 years of professional experience in physical therapy, specifically within geriatric care settings, assisted living, skilled nursing facilities (SNF), or home health.
Prior experience or knowledge of specialized areas such as fall prevention programs, post-operative recovery, or rehabilitation following strokes or joint surgeries.
